It also provides flexibility allowing employees to structure their available benefits to meet their unique needs and to change the structure annually as their needs change.

2017 Open Enrollment Period 

Fall is a season of many changes. Fall is also the time to carefully review your personal benefits and consider any changes during the annual Open Enrollment period. Open Enrollment allows you to take charge of your benefits by selecting the options most important to you and your family.

The 2017 Open Enrollment period is October 10-28. This is the only time of the year employees may make changes to medical and dental plans and enroll eligible dependents (except in the case of a change in family status). Changes will be effective on January 1, 2017. Employees will receive a personalized Open Enrollment statement with coverage, bi-weekly contributions and dependent information, effective January 1, mailed to their home address. Eligible CTA employees can enroll in Supplemental Term Life Insurance, without proof of good health, during Open Enrollment (restrictions apply).

Remember, if you do not wish to make any changes your current elections will continue for the 2017 plan year. Eligible employees with outside medical coverage must make an annual election to opt-out of CTA medical coverage. Documentation is required when opting out of CTA’s plans or when adding a dependent(s).  SEE REQUIREMENTS.

How to Make Your 2017 Elections

Open Enrollment elections (including opting out, if eligible) should be made using Employee Self Service beginning October 10. The last day to change elections is October 28. Please also ensure that your home address is current to receive all CTA Benefits Services mailings.

Come to an Open Enrollment Information Session

Healthcare vendors and the HR Benefits Services team have scheduled information sessions throughout the 2017 Open Enrollment period. Visit us at your worksite for plan and enrollment information, giveaways and the opportunity to win great prizes! The CVS/Caremark bus will also be onsite to administer flu shots (employees must bring medical/prescription card and a photo ID).

Important 2016 Income Tax Information

To comply with the Affordable Care Act (ACA), CTA is required to collect the social security numbers of every family member covered on an employee’s medical plan. Please enter this information in Employee Self Service on or before October 28. Employees will receive IRS Form 1095C showing which month(s) employees and dependents were enrolled for coverage. You will need the information provided on this form when you file your 2016 income taxes. IRS Form 1095C will be mailed to your home address no later than January 31, 2017. 

 

Take Charge of Your Health!

This year, as part of our Take Charge of Your Health campaign, employees that receive a qualifying physical and have their physician’s office complete the Physician Affidavit form will be entering into a weekly drawing to receive a $25 gift card and a grand prize drawing at the end of the campaign on Wednesday, November 30. This is our way of saying “thank you” for taking advantage of this great plan benefit and being attentive to your health.

Follow these three steps to “take charge of your health” and for a chance to win!

  1. Now through Nov. 30, schedule your annual physical - it is covered at 100% under any Cigna medical plan.
  2. Download the physician affidavit and take it with you to your appointment.
  3. Your physician’s office must send the affidavit to CTA Human Resources via email, fax or mail by Nov. 30.
     

Please Note: If you have already received a qualifying physical in 2016 you are still eligible to be entered into the drawings. Please have your physician complete and send the physician affidavit to Human Resources before Nov. 30.
