This fall, CTA Wellness is encouraging employees to take advantage of their FREE preventative screenings.
Take charge of your health and win!
Follow these 3 steps for a chance to win:
1. Now through December 7th, schedule your preventative screening. This screening can be with your:
- Primary Care Physician (annual physical)
- Urologist (prostate screening)
- Gynecologist (pap screening or mammogram)
- Dentist (dental exam and cleaning)
3. Have your physician fill out and submit said affidavit through one the following means by December 7th:

Each week, names will be drawn from received affidavits for a chance to win! If you have already received a qualifying preventative screening in 2019 you are still eligible to be entered into the drawing.
Please have your physician complete and send your affidavit to Wellness before December 7th.
